An unidentified man comes into the hospital with critical injuries after being struck by a subway train. The doctors rally together to try to save his life, but Charlie’s (Michael Shanks) medical struggle is soon outweighed by a spiritual one when he discovers the John Doe is Randal Crane (Peter Keleghan), who comes with a dire warning about Charlie’s gift and its heavy cost if he doesn’t rid himself of it, just in time for Charlie to discover that Alex (Erica Durance) is pregnant. Alex takes her mother (Jennifer Dale) to their family farm, where she learns not only that her mother is planning to sell the farm, but she is also planning to take her own life before the illness progresses to an unbearable degree. She asks for Alex’s assistance, but Alex refuses so she takes matters into her own hands and takes her own life. Maggie (Julia Taylor Ross) is on a quest to put her past behind her, but on cue, Sydney Katz (Stacey Farber) comes back into her life, with her pregnant sister who she wants in Maggie’s cancer study, not knowing that the study has been pulled from the hospital. Sydney admits that helping her sister isn’t the only reason she returned to Hope Zion and professes her love for Maggie. Zach (Benjamin Ayres) and Dev (Dejan Loyola) treat an elderly man, Wilfred Jennings (Kenneth Welsh), who is slowly trying to kill himself with drugs, alcohol, and fast living ever since his wife died, and must convince him that life is still worth living, even without her.
